Job Salary: Salary commensurate with experience ($36,670-$47,330); plus, Benefits

Purpose of Position: The primary goal of the Community Health Worker is to promote individual and community-wide health and well-being and housing stability for residents at DevNW-owned affordable housing sites. By reaching people where they live, DevNW is in a unique position to offer individualized short term case management, health promotion and specialized programming that targets the needs of low-income individuals. This position works collaboratively with the DevNW Thriving Communities team to coordinate and implement health, wellness and financial wellness programs designed to meet the needs of DevNW residents.

Essential Duties:

  • Community engagement & developing healthy communities –
  • Work in collaboration with community partners and residents to coordinate programming at DevNW properties that may include classes such as chronic disease prevention and management, community gardening, nutrition classes, healthy communities planning, parent education, child programming, neighborhood watch and leadership development.
  • Work with property management and asset management staff to build a healthy and engaged culture among residents at each of our properties, including leadership development for residents, conflict resolution, building trusted relationships, etc.
  • Resident Housing Stability and Health –
  • Build relationships with DevNW residents to set goals related to successful tenancy that includes, but is not limited to wellness, maternal and child health, chronic disease prevention and management, and/or financial wellness education.
  • Community Health Workers will meet one-on-one with individuals/families and follow up at regular intervals to assess progress, address barriers, and make changes to their plan as necessary. Responsibilities may include support in financial wellness classes and coaching and referrals to behavioral health specialists, assisting with health insurance enrollment, explaining health and wellness plans, and selecting a primary care physician, oral health referrals, harm reduction, and tobacco cessation.
  • Work with property management staff to provide additional support to tenants at risk of eviction, including providing individual coaching and conflict resolution.
  • During COVID, responsibilities may also include food access, technology support, provision of PPE, wellness checks, and other crisis supports.
  • Information and referral - Provide information and referral, advocacy and/or short-term case management services to DevNW residents in crisis or in need of social, educational and/or employment related services. Responsibilities focus on contributors to health including early intervention, food security, social service referrals including the need for emergency rent assistance, payment plans to stabilize housing, and utility assistance.

COVID – 19 Emergency Reponses:

  • Due to the pandemic the nature of CHW service delivery has moved from in-person direct to virtual service delivery. Our offices are closed until June 2021. All staff are required to maintain a safe work environment from home, until our office opens again. The CHW team connects with residents at a minimum quarterly, coordinates emergency resources and rent assistance, Familiarity with virtual tools such as YouTube, FaceBook, Google Drive, and Zoom to share resources virtually as a team, and connect to residents is essential. The pandemic and the wildfires, this year alone, have affected our residents and community in unpredictable ways, and the ability to adapt to new and changing crisis response is critical.

Required Knowledge and Skills:

  • Knowledge of and experience working (one-on-one and in groups) with diverse populations, preferably experience related to issues of poverty, leadership development, neighborhood improvement, health care, health equity promotion, public health and/or community development.
  • Knowledge of and experience with social service networks; especially services most likely needed by low-income individuals and families.
  • Experience providing information and referral, advocacy and case management services to individuals and families.
  • Ability to develop and maintain a cooperative and team-oriented working relationship with a wide range of people, including other employees, neighborhood residents, the general public, volunteers and others.
  • Ability to work independently and make critical decisions in the field. Excellent oral and written communication skills.
  • Ability to speak in public before groups of people. Bilingual in Spanish preferred, but not required.

Other requirements include:

  • Must have a valid and current Driver’s license and be fully insurable by our insurance provider to operate company vehicle for business purposes. Those who are not fully insurable, may be able to use own personal vehicle with current auto insurance coverage while on company business on a case by case basis.
  • Ability to use a computer, keyboard and associated software for accounting, word processing, spreadsheets, databases, email and calendar/scheduling programs.
  • Completing a criminal background check prior to being hired, the results of which are satisfactory to the employer.
  • Ability to work some evening and weekend hours; travel to other locations in Oregon (once a month or less); and travel out of state for training and/or conferences for periods up to a week (once a year or less).
